3. What about DHT and genetics?

  • DHT (dihydrotestosterone) shrinks hair follicles in people with genetic sensitivity (AR gene).

    Serum-DHT versus follicular-DHT – what's the difference?

For baldness, its influence is relatively small, as circulating DHT barely reaches the hair roots.

    • Follicular-DHT (in the scalp) is produced locally via the enzyme 5-α-reductase type II. Here, genetic factors (especially the AR gene) and local enzyme activity play the main role. This is the decisive form of DHT: in people with a predisposition to androgenetic alopecia, follicular-DHT causes the hair follicle to shrink progressively.

    Important to know: creatine does not affect any of the enzymes that convert testosterone to DHT. Only an extremely high loading phase of 25 g per day once showed a short-term increase in serum-DHT (the 2009 rugby study). Modern studies with the usual 3–5 g per day show no change in serum-DHT and no difference in hair density or minoxidil score.

    In short: even if serum-DHT temporarily increases, that does not mean more DHT enters the hair follicle. At standard dosages (3–5 g per day), there is no evidence of increased follicular-DHT and therefore no reason to link creatine to hair loss.
